Required Health Records Documentation
New employees must provide proof of Hepatitis B vaccination (or a declination form signed at hire) and a negative TB test performed within the past 12 months. A negative chest x-ray and no evidence of symptoms, or completion of treatment is also acceptable. A physical will be required if hired.
